After that date, we will no longer be seeing patients or providing medical services at this location. We encourage you to begin the process of establishing care with another licensed health care provider for your ongoing medical needs. If you have an upcoming appointment scheduled before the closing date, we will be happy to assist you with your transition during your visit.

Effective March 16, 2026, all patient medical records of Darien will be transferred to and maintained by an independent medical records custodian, Morgan Records Management.
You may obtain copies of your medical records or request that your records be sent directly to another health care provider by submitting a request to the custodian at the contact information below. Reasonable copying and mailing charges may apply as permitted by law. The custodian will maintain your records for at least the applicable retention period and will process requests in accordance with federal and state privacy laws.
